I. Rules on Handling of Personal Information

Unisis sometimes asks You* to provide Your personal information in various aspects of its business activities.
Unisis will make its best efforts to appropriately manage and protect personal information by complying with relevant laws, regulations, and guidelines, including the Act on the Protection of Personal Information, (hereinafter, the “relevant Laws and Regulations”) and Unisis’s internal rules (hereinafter, the “Internal Rules”) and by establishing this Privacy Policy (hereinafter, “this Policy”), as follows.

* In this Policy, the terms “You” and “Your” refer to Unisis’s customers and business partners, visitors to the hospitals which are Unisis’s customers (e.g., patients), shareholders, job applicants, and its officers and employees (hereinafter, “Officers and Employees”).

1. Acquisition of Personal Information

Unisis will appropriately obtain personal information in accordance with the relevant Laws and Regulations, and the company’s Internal Rules. If Unisis needs to obtain information designated by law as special care-required personal information, Unisis will, in principle, obtain such information with Your consent.

2. Purposes of Use

Unisis will identify, as comprehensively as possible, each purpose of use of personal information, in accordance with the relevant Laws and Regulations, and Internal Rules, and will handle such personal information within the scope of that purpose. Unisis will also notify or disclose, and if necessary, expressly present the identified purpose of use to You those concerned, in accordance with the relevant Laws and Regulations and Internal Rules. The purposes of use to be disclosed by Unisis are as stated in “II. Purposes of Use of Personal Information.”

3. Information Security and Erasure of Personal Information

Unisis will handle personal information in a manner that assures appropriate security. Unisis will also keep personal information accurate and up-to-date to the extent necessary for use and will erase such personal information as and when the information is no longer required.

4. Provision to Third Parties

Unisis will not provide any of Your personal information to any third party (including any overseas third party) without Your prior consent, unless expressly required to do so under the relevant Laws and Regulations. In cases where Unisis provides Your personal information to any third party, Unisis will take necessary and appropriate measures to ensure that the personal information is appropriately handled by the third party.

Unisis sometimes outsources acquisition, use, and other processing of personal information to contractors. Unisis will supervise such contractors in a necessary and appropriate manner.

Unisis sometimes uses cookies, etc. collected from its websites. Unisis uses such information to analyze Your trends and characteristics to be able to provide better services.

II. Purposes of Use of Personal Information

1. Personal Information of You and Unisis’s Business Partners

– Reviewing and analysis for the purposes of product and service quality improvement through customer satisfaction surveys, etc.
– Investigation and record-keeping under the relevant Laws and Regulations, and notification and reporting to public offices

2. Personal Information of Visitors to the Hospitals which are Unisis’s Customers (e.g., Patients)

– Research conducted by Unisis alone or collaboratively with hospitals or research institutions such as universities.
– Review of and analysis for product and service quality improvement through responses to product failures, etc.
– Investigation and record-keeping under the relevant Laws and Regulations, and notification and reporting to public offices
– Implementation of tests or analysis for medical institutions (when entrusted by them) and reporting of their results

3. Personal Information of Shareholders

– Stock management, including data preparation under a relevant law or regulation
– Responses to exercise of rights by shareholders under the Companies Act
– Fulfillment of obligations under the Companies Act and other relevant laws and regulations
– Delivery of business reports and other materials to be distributed
– Promotion of exercise of voting rights

4. Personal Information of Job Applicants and Internship Applicants

– Recruitment and internship selection information and procedures
– Responses to inquiries and document requests
– Reviewing and analysis of recruitment and internship selection activities
– Post-recruitment employment management procedures

5. Personal Information of Officers and Employees (including ex-Officers and ex-Employees) and Members of Their Families

– Management of human resources/labor, payment of salary/remuneration, evaluation, skills development, welfare, etc.
– To practice workplace safety for the management of health and safety as well as prevention of infectious diseases at work
– Allocation, relocation, and other employment management matters
– Communication, etc. with labor unions, health insurance societies, and pension funds
– Investigation, record-keeping under the relevant Laws and Regulations, and notification and reporting to public offices
– Communication in the event of an emergency
